The automated solution, which will be unveiled next week at the American Society of Health-System Pharmacists� (ASHP) Midyear Clinical Meeting in Orlando, Fla., combines prescription data from SureScripts-RxHub�s information network and a hospital�s own health information systems to generate a snapshot of each patient�s prescription medication history. �Having accurate prescription information for patients is fundamental to providing quality care. Yet, current processes for gathering medication history are inconsistent and error-prone. They leave the door open to ADEs,� said John Parmley, Standard Register�s director of Healthcare Risk Management programs. �Rx History Capture introduces greater accuracy and efficiency to the process by providing clinicians timely prescription information from trusted sources.� According to the Agency for Healthcare Research and Quality, over 770,000 people are injured or die each year in hospitals from adverse drug events. Moreover, a study released by the Brigham and Women�s Hospital and published in the Journal of General Internal Medicine (September 2008) states that 72 percent of potentially harmful medication discrepancies are due to errors in taking patient medication histories. Rx History Capture provides clinicians with a list of �as-filled� prescription medications which becomes an important validation tool in discussions with patients and family members. It reduces reliance on their recollections by providing vital details such as prescription dosage and strength. The objective data also helps to avoid confusion over similarly named drugs. �Rx History Capture will help hospitals improve patient safety in a fundamental way,� Parmley said. �It offers a consistent, reliable approach for gathering medication histories that will help enhance accuracy, create a solid foundation for medication reconciliation and facilitate responsive care.� Standard Register will be previewing Rx History Capture in booth # 925 at the ASHP Midyear Clinical Meeting, which opens December 7.
